Job Purpose
To work collaboratively with the Deputy Medical Director and all healthcare professionals to provide comprehensive Clinical Pharmacy Services and ensure the Medication management and use practices are safe and effective at Andheri Unit.
Key Responsibilities
- Service
- To work with the multidisciplinary team to enhance patient safety by ensuring the safe, appropriate, and cost-effective use of medicines for the patients. including therapies such as chemotherapy and immunosuppressants.
- To conduct audits of patients’ medical and medication records to recognize and discern medication errors, drug interactions, and adverse drug events; and also assess the potential risk & strategies to manage such risks and make clinical interventions to ensure the appropriate use of medications throughout the hospital.
- To conduct Drug Utilization Reviews (DUR) to study the medication usage pattern & process, ensuring appropriate use of medications.
- To detect, analyze, and prevent or manage any concerns associated with drug-related problems and to make appropriate evidence-based patient-centric medication therapy recommendations or practices and processes.
- To participate in interdisciplinary patient care rounds and intervene and communicate the follow-up about the therapeutic changes, if any, with the clinical team. including patients admitted for transplant as well as critical care areas.
- To assist HOD and other hospital team members in designing policies and procedures, ensuring safe, effective, and appropriate use of the medication management process.
- To discuss all the concerns about the medication management and use process with the relevant Committee members.
- To coordinate Pharmacy & Therapeutic Committee meetings every month.
- To participate in all interdepartmental meetings and committees and fulfill all the responsibilities as assigned.
- Prepare departmental MIS as per the calendar.
- Any other responsibilities as assigned by the HOD from time to time.
- Growth
- Participate in health programs in the hospital, Outreach activities or communities for prevention and treatment of injuries or illnesses
- Provide medication formulary support to new clinics and specialties for further growth of the hospital
- Quality
- To identify and report medication errors and adverse drug reactions to the Quality team, thus helping to improve the quality of care.
- Ensure quality assurance, quality improvement programs, and safety-related activities are adhered to across the department
- Ensure adherence to internationally accepted standards of care, including but not limited to those recommended by JCI, NABH, and other quality systems
- Support in documentation for accreditation activities.
- Finance
- To manage the Hospital Drug Formulary continually.
- To evaluate and provide inputs on new drug products, generic molecules, and formulations to the Pharmacy & Therapeutic Committee (PTC).
- To evaluate and provide inputs on any updates (like addition or deletion of drug products) in the existing Hospital Drug Formulary to the Pharmacy & Therapeutic Committee (PTC).
- To identify high-cost and high-use drug agents within the defined areas of practice and participate in financial management initiatives (like revenue generation, cost containment, etc.)
- To recommend generic substitutions and therapeutic interchanges without affecting the quality of patient care, to achieve the business plan.
- To provide suggestions/feedback to further improve Clinical Pharmacy Services and the growth of the Clinical Pharmacy department.
- Innovation & Research
- To share the details of Adverse Drug Reactions observed in the hospital with the Central Drug Standard Control Organization (CDSCO) under the National Pharmacovigilance Programme of India.
- Keep up-to-date about the latest developments in medicine products, the clinical field, and treatment strategies.
Qualification: PharmD or M. Pharm (Clinical Pharmacy), Registered Pharmacist in India is required for this Freshers Job.
Experience: PharmD: 0-3 years of clinical experience / MPharm: 3-6 years of clinical experience.
