About the Job
- Maintain all operations within NABH standards.
- Perform sterile compounding of anti-cancer drugs.
- Provide clinical medication education particularly in oncology for nurses.
- Prepare Pharmacy and Therapeutic committee meeting agenda, information and circulate minutes.
- Prepare antibiotic utilization data and present in Hospital infection control meeting.
- Perform regular Crash cart audit, Medication cart audit, Narcotic audit.
- Review patient records and enter drug orders using eMAR.
- Perform drug utilization review of all medication charts and note any discrepancies.
- Compile detailed report of medication errors and ADR’s noticed and present the same in Pharmacy therapeutic committee meeting and prepare strategy for preventive actions.
- Present detail monthly report of NABH – Quality indicators.
- Ensuring adherence to restricted antibiotic policy.
- Perform drug utilization review, antibiotic stewardship, Pharmacokinetic monitoring.
- To give regular rounds to all patients, who are getting chemotherapy in day care facilities as well as inpatient.
- Oversee that chemo protocols are properly followed on each case basis and act if deviations are there.
- Proper monitoring of patients getting chemotherapy.
- Adverse Drug Reaction (ADR) detection, reporting, monitoring, documentation and assessing suspected ADR’s by using different causality assessment scales.
- Provision of drug and poison information.
- Counselling of patients on discharge medications.
- To lead team members in ensuring all responsibilities of department are completed within defined time frame.
- To manage roster/leave/attendance of all team members and ensure adequate coverage.
- Supervise all activities of department to ensure minimum or no error and take CAPA measures appropriately.
